Federal Contractors Must Use Revised Voluntary Self-Identification of Disability Form
On April 25, 2023, the Office of Management and Budget (OMB) approved the revised Voluntary Self-Identification of Disability Form (CC-306) issued by the Office of Federal Contract Compliance Programs (OFCCP). Contractors must implement the new form no later than July 25, 2023.
What is the Voluntary Self-Identification of Disability Form? All federal contractors or subcontractors are required to provide equal employment opportunities to qualified individuals with disabilities. Among other requirements, federal contractors and subcontractors are required to give this form to employees to invite them to voluntarily self-identify whether they have a disability.
What are the changes in the revised version? OFCCP changed the formatting and revised the language of the form to make it more readable. They also added several listed disabilities, for example:
- “Alcohol or other substance use disorder (not currently using drugs illegally)”
- “Mobility impairment, benefiting from the use of a wheelchair, scooter, walker, leg brace(s) and/or other supports”
- “Neurodivergence, for example, attention-deficit/hyperactivity disorder (ADHD), autism spectrum disorder, dyslexia, dyspraxia, other learning disabilities”
- “Pulmonary or respiratory conditions, for example, tuberculosis, asthma, emphysema”
- “Traumatic brain injury”
What must federal contractors or subcontractors do to comply? Start using the new form for employees and applicants no later than July 25, 2023. Federal contractors and subcontractors are required to provide the Voluntary Self-Identification of Disability form to all employees every 5 years.
